Steps to Sign Up for FORMED:

2.2. Under “Create a New Account” to find your Parish or Organization type in the zip code “54401”
3. When “St. Matthew Parish” shows up, click on it to select it, then click on “Next”4.
4. Create a New Account by typing in “Your Name” and “Your email address” then click “Sign Up”
